It must be defeated before it is possible to defeat Heart-rot because of the Twisted Heart's Blessing that is applied on Heart-rot. However, to be able to damage The Twisted Heart its roots must be destroyed, and it grows those down the cave near Heart-rot. Thus, let Longbough tank Heart-rot and fight The Twisted Heart until it plants more roots, quickly run down and destroy them and then up again.
