When 
Disorientation is traited, the following skills have a chance to apply this effect to the enemy target(s):
Shield-blow
Shield-smash
Cataclysmic Shout (guaranteed, requires original trait)
The trait 
Insult to Injury makes your damaging skills have up to a 20% chance to refresh this debuff.
When an enemy target is within 
Bastion of Light, the Guardian's damaging skills have up to a 35% chance to spread this effect on up to 3 other targets within the hotspot.
The trait 
Disabling Strikes increases the potency of the effect.
