This effect appears on yourself inside Barad Guldur, during the Lieutenant of Dol Guldur-fight, as soon as the fellbeast is chasing you (you wel get a yellow-eye symbol above your head). It means that you should kite the fellbeast around, as soon as it gets to close, it will bite you resulting in an instant dead!
